1.	Employee's Social Security Number.
2.	Employee's name.
3.	Pay period and check date.
4.	REGULAR - regular semi-monthly pay rate (annual salary divided by 24 pay periods).
5.	IMPLIFE - imputed life insurance (if life insurance amount is over $50,000, this amount is reported as taxable).
6.	FED TAX - Federal income tax withheld (based on current earnings).
7.	OASDI - Old Age Survivors Disability Insurance: disability portion of social security (6.2% of current earnings).
8.	HI - Medicare portion of social security (1.45% of current earnings).
9.	VA TAX - State of Virginia tax withheld (based on current earnings).
10.	PRM CONV - health insurance premium.
11.	POST TAX - optional disability insurance.
12.	USBONDS - optional benifit to purchase United States Bond Certificates.
13.	DDCHKING - direct deposit into employee's bank account.
14.	VACU - optional state employee credit union.
15.	PRE TAX - optional tax sheltered annuity.
16.	CVC - optional charitable contribution.
17.	DEBITCRD - optional Radford University debit card.
18.	SSPLMNT - salary supplement (for employees who completed work for the University other than their salary contract agreement).
19.	SUMSCHL - earnings for summer school teaching.
